Every chapter is expected to submit an Annual Report to the Executive Secretary in the fall. Failure to file such a report may lead to deactivation of the chapter.

A Report on Initiates  should be submitted to the Executive Secretary whenever a chapter initiates new members. Please note that a candidate is not officially a member of Eta Sigma Phi until such a report is submitted.

A Chapter Report summarizing the activities of the chapter during the current academic year should be submitted by April 30 of every year.

Each chapter is encouraged to acquire its own Tax Identification Number (TIN) from the IRS. This is done by completing Form SS-4 and submitting it to the IRS.
