Forms and Reports

Every chapter is expected to submit an Annual Report to the Executive Secretary in the fall. Failure to file such a report may lead to deactivation of the chapter. Any chapter not submitting this report by Nov. 15th will receive only one subscription to the NUNTIUS for that academic year.

A Report on Initiates  should be submitted to the Executive Secretary whenever a chapter initiates new members. Please note that a candidate is not officially a member of Eta Sigma Phi until such a report is submitted.

A Chapter Report (Res Gestae) summarizing the activities of the chapter during the current academic year should be submitted by May 31 of every year.
