Please complete this report and return it as soon as possible after each initiation to the National Office.  You can submit this report on-line by completing this form or you can mail a print copy (Report on Initiates in .pdf) to the Executive Secretary. The names of initiates must be typed as they should appear on the membership cards and certificates.  The fee schedule is listed below.  Thank you for your cooperation.  If you submit this form on-line, please print out a copy to enclose with payment. If you use a print copy, please print or type each name clearly and list names in alphabetical order.

National Fees are:

Initiation fee for active membership, consisting of undergraduate students duly initiated by a local chapter (including a certificate and a subscription to Nuntius)
 		  $30 per initiate
Initiation fee for associate membership, consisting of graduate
students in Classics elected under the requirements of membership     
     		$10 per member
Members elected honoris causa by local chapters
 		  No fees

Optional membership certificates for associate and honorary members
           		$10 per certificate
