The registration fee includes all meals on Saturday, including the banquet, plus breakfast on Sunday. Delegates will be on their own for dinner on Friday night and for  lunch on Sunday.

Registration options:
 

1.       Mail in: $70 per person. Use this printable registration form and mail with ONE check, made payable to Eta Sigma Phi. to Dr. Thomas J. Sienkewicz, Executive Secretary, Monmouth College, 700 E. Broadway, Monmouth, Illinois 61462.Must be postmarked by 3/10/08. No mail-in registrations will be accepted after that date.
2.       Or use this form to register on-line. Cost $75 per person (includes a handling fee to cover the fees for this on-line service). Payable via PayPal only. The cost of on-line registration will increase to $80 per person after 3/10/08.

Note: Members submitting proposals to be presented at convention must pay their registration fee by 2/1/08.

All registration fees are non-refundable.
