 Registration Options and Instructions

The registration fee includes all meals on Saturday, including the banquet, plus breakfast on Sunday. Delegates will be on their own for dinner on Friday night and for  lunch on Sunday.

Registration options:
 

1.       Mail in: $85 per person. Use this printable registration form and mail with ONE check, made payable to Eta Sigma Phi. to Dr. Thomas J. Sienkewicz, Executive Secretary, Monmouth College, 700 E. Broadway, Monmouth, Illinois 61462.Must be received by 3/15/11. No mail-in registrations will be accepted after that date. After March 15th only on-line or on-site registrations will be accepted.
N.B.: There is no registration fee for one faculty chapter sponsor accompanying student delegates to the convention. This registration fee waiver is valid only until 3/15/11.
 

2.       Or use this form to register on-line. Cost $90 per person (includes a handling fee to cover the fees for this on-line service). Payable via PayPal only.
The cost of on-line registration will increase to $95 per person after 3/15/11.

  Acceptance Mark

All registration fees are non-refundable.
