The
Illinois Post-Secondary Educational Institution Immunization Law (Public Act 85-1315) requires
that all new students show documentation of immunity to tetanus/diphtheria, measles, rubella
and mumps before they can register for classes.
IMPORTANT: You will not be permitted
to enroll in classes or move into your assigned residence hall until Monmouth College receives
ALL immunization documentation.
We
have included the "Notice
of Immunity Requirements" for all students and the Illinois Department of Health -
"Certificate
of Immunity" Forms online in Adobe PDF format. If you do not have
Adobe Reader on your
computer, you can download a free copy from Adobe.
Checklist of State Requirements:
1. Have your family
physician complete the "Certificate of Immunity". All dates and signatures must be filled in
for it to be valid.
2. Illinois residents
may submit their school health form "Certificate of Child Health Examination". ALL
immunizations listed above MUST be listed. If not, obtain other signed documentation for other
immunizations.
3. Students born
before 1-1-1957 are EXEMPT from these requirements. Other exemptions are listed on the
"Certificate of Immunity".
4. Return the
"Certificate of Immunity" form as soon as possible so you can proceed with enrollment. Send
